Eligibility by Zip Code for PACE Gretna

To be eligible to participate in the PACE Gretna Program, individuals must be age 55 or older, qualify for nursing facility level of care in the state of Virginia, be able to live safely in the community with the support of PACE and live in a PACE Gretna service area as defined below.
